  • Download and print the forms below including the Registration Form, optional E-Mail Agreement Form and Health History Form; fill them out and bring them to your first appointment.
  • The initial visit will last about an hour.  In a comfortable, living-room-like setting, you and the doctor will discuss what brought you here and sufficient information about your life to have a good initial understanding of the problem.
  • Dr. Rivers-Bulkeley may want to obtain records (with your consent) from other doctors or providers whom you have seen, and may recommend medical tests or psychological tests if needed.
  • Dr. Rivers-Bulkeley will present his impressions and recommendations for discussion, and you and the doctor will agree on a treatment plan.  This can often be done at the end of the first visit, although sometimes 1 or 2 addtional sessions will be necessary to obtain further information from the patient or family members.
  • Subsequent appointments will be determined by the treatment plan that you and the doctor working together have created.

PATIENT PRIVACY PROTECTION ADVISORY
LOCKED PADLOCK OPENS WITH A KEY

The cornerstone of the relationship between the patient and the psychiatrist is trust.  Dr. Rivers-Bulkeley believes that confidentiality is essential for the development of trust, and he is a tireless advocate for its protection.  You have probably heard about "HIPPA" (Health Insurance Portability and Accountability Act) which was enacted in 2002 to impose complex, uniform Federal laws and regulations governing patient medical records.  Unfortunately, most consumers are unaware that in many regards it compromises patient confidentiality.  Click on the Padlock Icon to view our Patient Privacy Protection Advisory that explains why we have remained a non-covered entity.

OFFICE HOURS

Office hours are by appointment only (no walk-ins) and generally Monday through Friday from 9:30a.m. to 7:00p.m.  Limited Saturday morning hours are available for the convenience of patients who are unable to schedule a weekday appointment.

OUT OF HOURS CONTACT

Dr. Rivers-Bulkeley believes that access to the doctor is essential.  He can be reached after hours by telephone for matters that cannot wait until regular business hours or the patient's next scheduled appointment.  E-Mail can also be used for communication that is non-urgent once a patient is established in the practice and has signed the E-Mail Agreement.
